This facility doesn’t deserve a star, but I can’t proceed with a review unless I give at least one. This place should be closed. The staff is incompetent and lazy. They overdosed my mother with Insulin by giving her almost double her normal dosage then tried to cover it up. Her personal belongings would dissapear all the time without an explaination. If you love your family member don’t send them here.

My husband has been in several skilled nursing facilities in theme last couple of years & he has done better here than any other place. The staff is great, the facility is clean & it’s very close to home for us!

I have been at St Francis for just under 2 years. I love the atmosphere here. They have provided great care for me. I would recommend anyone to choose St Francis as their Skilled Nursing Facility.

Good people

The St Francis facility and staff are amazing. My dad had two stints here due to health circumstances, but fortunately the team here made it as easy as possible. The facility itself is nestled in the general neighborhood. Parking is simple as they have a large lot in the back. Upon entry, the area it bright and clean. Each room is alway clean and the building is properly staffed. In fact the staffing to resident ratio seem great. There is always someone to ask for help when necessary. Meals are complete and has a good variety. Caretaking of the patients is on schedule and the staff are friendly. The level of engagement with the patient is also great. My dad’s mental state is diminished, but the stuff still finds ways to interact with him. I often visit my dad and find that he is in a group activity that ranges from bingo, karaoke, and watching movies to name a few. I want to give a shout out to our case manager, Cynthia, who is always supportive and respond quickly to questions that we may have. Also to my dads physical therapist, Dominic, who always have a friendly demeanor, understanding tone, supportive nature, and most importantly patience to try and get my dad back to baseline. We also sincerely appreciate all the care and attentiveness to all the nurses that provides care to my dad around-the-clock. I know the staff is always watching because I get a phone call every my dad falls. I highly recommend St. Francis to anyone who needs to seek out a care facility.

I am formally expressing my dissatisfaction with the care my brother received at St. Francis Rehabilitation Center in Hayward. After being discharged from San Rose Hospital for heart failure, he was admitted to this facility for recovery. Despite the staff being generally polite, the conditions at the facility were extremely poor. My brother was placed in a crowded room with two other patients, limiting his privacy and comfort. Nurses only checked on him once a day, and I never witnessed them administering his medication during my visits. He was often left alone in his room for extended periods, which raised serious concerns about the attentiveness of the staff. Additionally, the rehabilitation services were grossly inadequate. He was given only 10 minutes of physical therapy per day, after which he was immediately returned to bed. This minimal effort does not align with the purpose of a rehabilitation center, which is supposed to focus on patient recovery and mobility. Based on this experience, the facility felt more like a hospice care center rather than a rehabilitation center. I believe patients and their families deserve transparency and proper care when entrusting their loved ones to a medical facility. The conditions and lack of adequate rehabilitation at St. Francis Rehabilitation Center are unacceptable. I hope improvements are made to ensure patients receive the care they need and deserve.

Everyone is nice and attentive. The food is good and the facility is clean.

I have the utmost confidence in this place. Clean. Excellent care. I have a non gluten non lactose free diet not by choice. They work around that they make sure until they get it right. You will have the utmost care and a very clean atmosphere. Very caring.

Everyone please listen carefully. These people have changed my life. God knows what I’m writing. please everyone take chance with these people they will change your life. If I could give them 10 stars, I would. So please check them out. Victor navin Singh, thank you.

My mom was admitted to this facility for recovery after she broke her hip. Going in, I wasn’t so sure about many of the choices we had since we were asked to make a choice within a day. Fortunately, St. Francis was above and beyond what we anticipated. The staff here were helpful with my moms need despite the language barrier. There was someone available every step of the way and that made my mom and me felt more secure. I would highly recommend St. Francis for these reasons.

We moved from Windsor to this facility as it was closer for us and little did we know it’s owned and managed by the same company, that’s why the service and commitment was identical. The caring nurses and management team was supportive, lot of resources and the compassion. You guys 🤘

Looking for a good rehabilitation facility? You found it. Great attentive staff. I speak from personal experience. If I need the use of the facility again, I will ask to be sent here.

This is a great facility! Because the facility is smaller it has a family feel and allows you to have a good relationship with your nurse and CNA’s.

May 2, 2021 @7pm PST, Tried calling several for my mom who is admitted at the facility but nobody is picking up the phone and the voicemail is full. 1-Star for this week. May 8,2021-My experience this past week was far better than the week ending May 2, 2021. SHOUTOUT to the clinical liaison team for going out of their way to address any feedback we have in regards to our mom’s condition who has a very high risk of falling(her dimentia only makes it worse). The staff are better coordinated and left nothing to chance by adding the good old bell to supplement a call Light and pressure type alarm to ensure my mom doesn’t walk to the restroom by herself. While there are still areas of improvement e.g. picking up incoming calls, I now feel more confident this will be resolve in no time. I will again share my experience for this coming week or so and hopefully the staff can sustain their momentum.

The new ownership and management have turned this into a fantastic healthcare facility. They work hard to ensure that everyone who comes in has a productive stay. I would highly recommend their services.

This place is horrible. The employees that run this place have zero follow through. I asked them to email me; I never received anything. My mother FELL and broke her hip and they left her sitting like that. It took them FIVE HOURS to get an ambulance. We were lied to about the kind of care that this horrible place provides. We were told this could be permanent; turns out that their only concern is to turn beds. When asked questions about certain conditions, they had NO IDEA how to answer any of it. This place is a joke. A bad one. If you care about your loved ones, avoid sending them here. They yell at the elderly and ignore them and smile in your face.
